It’s very important that you and your employer complete and sign the Loss of Salary Form  and return it to your attorney to make a claim for lost wages.

Keep a record of all the days that have to be taken off work due to your accident or injuries.

You must go to all of your doctor’s appointments.

Always tell your doctor if you are in pain. Be sure that you provide the name, phone number, and addresses of all the doctors treating you. If your

Accident requires any hospital stays, be sure to notify your attorney as soon as possible.

Keep track of all expenses relating to your accident,

including but not limited to; transportation costs, prescriptions, and other out-of-budget expenses.

WITNESSES

Provide the name and contact information of all witnesses to your attorney immediately.

If your injuries require a cast, neck brace, or other medical equipment, you must wear it until your doctor tells you otherwise. It is very important that you comply with your doctor’s orders. Please notify your attorney of your required treatment for your case file.

Provide your attorney with photos of your car, the scene of the accident, bruises, scars, or any other injury and damage resulting from the accident as soon as possible.

Notify us immediately of any address, phone number, or workplace changes. Please keep your attorney and the team informed of times that you will not be available and provide the office with your emergency contact’s name and number should we need to reach you.
